Wellness Benefit Policy - Benefit explanation and requirements
Wellness Reimbursement Form - ASD Wellness purchases must be submitted by November 9, 2018, February 8, 2019 or May 10, 2019. This form can be submitted quarterly beginning in November of each benefit year. Original receipt must be attached and submitted to Accounts Payable.
Wellness Waiver - This waiver must be submitted with the reimbursement form in order for you to be reimbursed.
